Information you give us
Request form. When you use the form on our Contact page we collect what you enter: your name, company, phone number, email address, the type of test and the reason for it, the test location, your preferred date and time, and anything you add in the notes field.
Phone calls. When you call dispatch we collect the information needed to schedule and perform the service you request.
Please do not include Social Security numbers, medical records, test results or other sensitive personal information in the request form. It is for scheduling only.
Information collected automatically
Our hosting provider keeps standard server logs that can include your IP address, browser and device type, the pages you request, the referring page and the date and time of each request. We use these logs to keep the site running and secure.
To block spam and automated abuse, the site uses CleanTalk anti-spam and security services. They check form submissions and visit information such as your IP address, browser details and the email address you enter. The request form also includes a hidden field and a timing check that help identify automated submissions.
Cookies and similar technologies
This website does not use analytics or advertising cookies, and it does not load advertising or tracking pixels. Our security and anti-spam services may set cookies that help tell real visitors from automated traffic, and site administrators use cookies to stay signed in. Some pages may request fonts from Google Fonts, which means Google’s servers receive your browser’s font request.
You can block or delete cookies in your browser settings. If we add analytics in the future, we will update this policy first.
How we use information
- To respond to your request, confirm the details and dispatch a collector.
- To communicate with you about scheduling, reporting arrangements and invoicing.
- To keep the records required to run our business and, for regulated testing, to meet record-keeping rules.
- To protect the website and our systems from spam and abuse.
We do not sell your information, and we do not use it for marketing lists without your consent.
Who receives information
We share information only as needed to deliver the service you asked for and to run the website: our web hosting provider (NameHero), where the site and its database are hosted; Microsoft 365, which delivers form notifications to our staff mailbox; CleanTalk, for the spam and security checks described above; and, once a collection is scheduled, the laboratory, Medical Review Officer and employer representative or ordering party that your testing program designates.
We may also disclose information when required by law.
Testing records and confidentiality
Information created during a collection, such as custody and control forms, alcohol testing forms and laboratory results, is handled under the rules of the applicable program, including 49 CFR Part 40 for DOT-regulated tests. It is released only to the employer representative, Medical Review Officer, ordering party or individual entitled to receive it.
Test results are not collected or stored by this website. Results are released through laboratory portals and the reporting arrangements of your testing program.
Phone calls and recordings
Telephone calls are recorded for training purposes, and callers hear a recording notice when calling our number. Access to our results email account, laboratory portals and call recordings is limited to Michael Baldwin and Stacy Baldwin, his wife and business partner.
How long we keep information
- Request form submissions are stored in the website’s database until we delete them. Access is limited to authorized administrators of the website.
- A copy of each form notification email is kept in the website’s email log for up to 90 days. Copies in our mailbox are kept as long as we need them to respond and to maintain business records.
- Email-stored test results and call recordings are retained for up to five years.
- Chain-of-custody records are retained according to applicable DOT requirements, and other testing records for the periods required by the applicable program rules.
Security
The site is served over HTTPS, form submissions travel over encrypted connections, and administrative access is limited to authorized staff. No method of transmission or storage is completely secure, but we take reasonable steps to protect the information you send us.
Your choices
You may ask what information we hold about your request, ask us to correct it, or ask us to delete it where we are not required to keep it: call 1-800-539-4934 or email ma*****@*********************ng.com. You can also skip the form entirely and call us instead.
Children
This website is intended for adults, and we do not knowingly collect information from children under 13 through it. Collections involving minors are arranged by a parent or guardian.

Pages on this site may link to other websites, such as laboratory or regulatory resources. Those sites have their own privacy practices, which we do not control.
